To Complete a Living Will

  1. Use a form from the state of which you are a resident.
  2. Make certain you sign your Living Will and have it properly witnessed. Two witnesses are required. In addition, there are specific limitations under the law on who is qualified to be a witness.
  3. Give a copy of your Living Will to your doctor and any other health care providers such as your hospital, nursing facility, hospice, or home health agency. In addition, you may want to give copies of your Living Will to other persons such as close family members and an attorney.
  4. DO NOT keep your only copy locked away. For your instructions to be followed, your Advance Directive needs to be available to your family and your health care provider.
