Dialysis Team
It is the commitment of MeritCare Dialysis to provide an individualized plan of care for every dialysis patient. This is accomplished by means of a specialized team of caregivers.
Social Worker
The social worker will assist the patient and his/her family to identify financial needs and psychosocial issues. If it is appropriate, the social worker will make community referrals for assistance with home care, meals, and living arrangements. The social worker is also responsible to assist patients with financial paperwork and referrals. Lastly, the social worker can also refer patients to appropriate dialysis facilities if they plan to travel.
Dietitian
The dietitian is responsible to assess your nutritional needs and make recommendations to you about your diet.
Nephrologist/Nephrology Physician Assistants
The nephrologist is the physician who specializes in kidney disease and dialysis. The nephrologist will be responsible to assess your physical needs and order appropriate medical treatment based upon their assessment. The physician assistants work with the nephrologists to make the assessments and order medical treatments.
Nursing Staff
The nursing staff will provide dialysis treatment and assess how a patient tolerates the treatment on a daily basis. They also assess overall needs and compliance to the treatment plan. The nursing staff is made up of registered nurses, licensed practical nurses, dialysis technicians, nursing assistants, and unit clerks.
Medical Director and Dialysis Services Manager
The medical director and manager of the dialysis unit are responsible to assure that the quality of care provided meets or exceeds the expectations of MeritCare Health System and the regulatory standards which guide the care of dialysis patients.
Pharm D
The MeritCare Renal Pharmacists provide for the management of anemia by assessing and ordering of treatment medications. They also assist the providers in managing dialysis related infections, renal bone disease, and medications in dialysis patients.
