MeritCare's Community Care Program offers options when medical bills become unmanageable. If you are unable to pay for required services provided by MeritCare, are not eligible for assistance from other public-funded programs and meet the criteria of this program, you may qualify to have all or a portion of your bill waived.
Covered Services
All MeritCare services are eligible for the Community Care Program with the exception of point of service co-pays, cosmetic procedures, non-covered services, eyewear, hearing devices and other retail items.
Eligibility Requirements
MeritCare strives to meet the needs of all patients. To determine if you are eligible for the Community Care Program, the following information will be considered:
- You must reside in MeritCare’s service area in Minnesota, North Dakota or South Dakota.
- Ability to pay for required services.
- All other available payment resources, including medical assistance, have been exhausted.
- Completion of a financial statement showing your income, assets and expenses along with supporting documentation.
Application Process
You may submit an application anytime before, during or upon completion of your stay/treatment.
To obtain an application, please call MeritCare Patient Accounts at (701) 234-2119 or (800) 263-2237. Applications are available at any financial counselor's office at all Patient Financial Service locations. Any financial counselor can assist you in completing the application.
Review and Notification
After completing your application, please mail it to MeritCare Patient Accounts, PO Box MC, Fargo, ND 58122-0793. Applications may also be delivered to any MeritCare financial counselor.
Within 15 business days of receiving your completed application, MeritCare will review it and appropriate notification will be mailed to you.
If you do not qualify for the Community Care Program and are unable to meet MeritCare's payment guidelines, extended payment plans may be available.
